What Can Go Wrong?
Two things go wrong with every chat system, and neither one is a malfunction. The context window fills up and older details fall out, and when grounding runs out the model fills the gap with plausible-sounding fiction called hallucination. Both have the same medicine: manage what is in the window.

- You can explain why a model forgets: the context window is a fixed-size buffer, and older turns fall out.
